FAMILY LIAISON PERSON

JOB TITLE Family Liaison Person

HOURS Part Time: 2.5 days per week

PURPOSE OF THE ROLE

The Family Liaison Person will be a direct point of contact for the
people we support and their families. Additionally, they will support
families to plan, organise and access a range of practical and
personalised supports and services, which enhance their participation
in, and contribution to, their local community. They will also be
involved in Advocacy Committee duties.

ROLE

* Facilitating the communications process for families and relatives
of the people we support acting as a point of contact for families in
order to provide advice, information, and support.
* Advocating for people we support in our Residential and Day
Services,?through communication, negotiation and representation of
the resident's and day attenders rights underpinned by SOS Kilkenny
values (Dignity, Diversity, Self-Determination and Inclusion).
* Assist, where needed, Social Workers for Residential / Day Service
for Admissions, ensuring responsiveness to Admission requests through
well-defined and transparent systems adhering to the Admissions,
Transition / Transfer, Temporary Absence and Discharge Policy of SOS
Kilkenny clg.
* Involvement with the SOS Kilkenny clg Complaints Procedure (where
appropriate) and SOS Kilkenny clg Open Disclosure Policy, in
particular liaising with the relevant stakeholders (as required) in
respect of issues so they can be resolved promptly at local level.
* Respite ? contact people availing of Day Services and their
families to offer Respite services if available.
* Assist in establishing Family Forum Meetings.
* Contact families where supports would be needed and where there
are changing needs.
* Advocacy presentations (including updates on the development of
the Capacity Legislation and infrastructure)
* Progression of the advocacy committee and service wide advocacy
including facilitation of the Monthly Advocacy Committee in
partnership with the Chairperson and Secretary of the Committee.
* Facilitation of focus groups for people we support re: service
deliver plan, etc,
* Linking with the family forum
* Record keeping, minute taking and report writing duties.

This is not an exhaustive list of duties and responsibilities, and the
post holder may be required to undertake other duties which fall
within the grade of the job, in discussion with the line manager.

QUALIFICATIONS

* A QQI Level 7 qualification in the area of Community Development,
Community Care, Disability Studies, Social Studies or equivalent.

SKILLS/EXPERIENCE REQUIRED

* Minimum two years in a similar role (Preferred but not essential)
* Excellent communicator and Listener
* Ability to collaborate with all stakeholders including families
and external community and education organisations
* Organised with attention to detail, Initiative and ability to work
to strict deadlines
* Adaptable to different environments and situations
* Knowledge of HIQA and awareness of Safeguarding
* Proficient in Microsoft Office
